The Agawam Health Department will NOT be offering COVID-19 Booster Vaccinations. COVID-19 Booster vaccinations may be available at our local pharmacies by appointment.
As of September 26, 2023, every U.S. household can order four free at-home covid test kits from the federal government at the link below.

- Drop-off and purchase of new 2 gallon ($10) Sharp's containers can be made between 9 a.m. – 4 p.m. 786-0400 X 8207.
- Appointments for Flu vaccinations can be made by calling Public Health Nurse at 726-9722.
- Appointments for all septic plan reviews, perk tests or general septic information can be made by calling: Health Agent @ 726-9721.
License fees and downloadable applications can be found on the left side of our webpage for your convenience.
*New August 25, 2023: In accordance with the Town of Agawam Chapter 149 Tobacco Ordinance, we have reached our cap of thirty (30) Tobacco Sales Product Permits and cannot issue any additional permits at this time. Applicants who have already been issued a permit will not be affected. Check back for updates or contact the office.

Duties & Responsibilities
The Health Department is responsible for providing educational and personal services to the residents in order to improve the public health of the community. It is also responsible for the provision of public health services in the Agawam Public Schools via the school nursing program.
