The Public Health Nurse focuses on population health through continuous surveillance and assessment of multiple health determinants to promote health and wellness, prevent disease, disability and premature death, and improve the quality of life. The nurse will address population health priorities through identifying, implementing, and evaluating universal and targeted evidence-based programs and services that provide primary,secondary, and tertiary prevention interventions.
Services
- Blood Pressure Screenings- by appointment in the Health Department or at the Agawam Senior Center on the 2nd and 4th Thursday of each month
- B-12 injections: Provided to housebound residents or by appointment in the Health Department, must provide physician's order and medication
- Vaccinations/ Immunizations: Cost may vary, call 413-786-0400, ext. 8207 for more information
- Limited dressing changes: Must provide written physician order
- Home visits and Referrals
- Complaint and Communicable Disease Outbreak Investigations
- Conducts Health Fairs and Educational Programs
- Blood Sugar Screening: Cost $5 by appointment
- Mantoux-Tb testing: Cost $10 Administered on Mondays - read on Thursdays, call 413-786-0400, ext. 8207 for an appointment
